Personal Health Partners is seeking a Nurse Practitioner (Adult/Gerontology) to provide high-quality primary care in a high-touch, resident-focused model at Clay Healthcare Center in Ivydale and Braxton Healthcare Center in Sutton, WV.

The NP will diagnose, treat, and manage acute and chronic conditions, collaborate with the care team, and ensure accurate documentation in our EMR. This full-time role offers a Monday–Friday schedule with a $15k sign-on bonus and quarterly productivity bonuses.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification of AANP or ANCC.
• Active, unrestricted state license; Active DEA license.
• US work authorization and 2+ years clinical experience in geriatric/adult/family practice.

Responsabilités

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education and develop treatment plans.
• Collaborate with PCP and interdisciplinary care team; coordinate care and telemedicine as needed.
• Maintain compliant documentation in EMR and meet regulatory requirements.
