Job summary

Curana Health, Inc. is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based care to seniors in skilled nursing and assisted living communities. You’ll practice at the top of your license with support from physicians and a strong clinical team.

This role emphasizes autonomy, collaborative care, and the use of advanced technology to improve outcomes, reduce hospitalizations, and enhance patients' quality of life in a mission-driven organization.

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations and current board certification.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.

Responsibilities

• Deliver high-quality primary care including comprehensive health assessments and individualized treatment plans for residents in multiple living communities.
• Diagnose, treat, and manage acute and chronic conditions with focus on preventative care and long-term wellness.
• Provide ongoing care management to improve quality of life while promoting independence.
• Build meaningful patient relationships through regular facility rounds and proactive interventions.
• Collaborate with physicians, clinical staff, facility leadership, and families to coordinate care plans.
