Job summary

Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality primary care across senior living and skilled nursing facilities. You will assess residents, manage chronic conditions, and collaborate with physicians and care teams to optimize outcomes.

You'll practice with autonomy in a mission-driven organization that leverages advanced technology and analytics to improve patient care and reduce hospitalizations. Travel to assigned facilities is required.

Qualifications

• Valid NP/PA license in the state of operations and current board certification.
• Masters Degree in Nursing or PA Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with focus on preventative care.
• Provide ongoing care management to improve quality of life and independence.
• Build meaningful patient relationships through regular facility rounds.
• Coordinate care with physicians, staff, and families to develop comprehensive plans.
