Curana Health seeks Nurse Practitioners and Physician Assistants to deliver proactive, relationship-based primary care to seniors in our community-based facilities. Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
• Masters Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships with residents, families, and care teams.
• Collaborate Across the Care Continuum with physicians, clinical staff, facility leadership, specialists, and family members to coordinate care.
• Maintain accurate EMR documentation and ensure compliance with regulatory, clinical, and organizational standards.
