Qualifications

• Valid NP or PA license in the operating state and current board certification.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships; conduct regular facility rounds with residents, families, and care teams.
• Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations.
• Educate residents and caregivers on disease management, preventative care, and wellness strategies.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and family members to coordinate care.
• Maintain accurate and timely documentation within the EMR and ensure compliance with standards.
