Job summary

Curana Health is seeking Nurse Practitioners and Physician Assistants to deliver proactive primary care directly to seniors in skilled nursing, assisted living, and independent living communities. You will build lasting patient relationships, practice with autonomy, and work with experienced clinicians and advanced technology to improve outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Current board certification in the respective field.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication abilities.

Responsibilities

• Deliver comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management to improve quality of life and independence for residents.
• Build meaningful patient relationships through regular facility rounds and collaboration with care teams.
• Coordinate care with physicians, clinical staff, leadership, and families to implement care plans.
• Ensure compliance with regulatory and organizational standards and maintain accurate EMR documentation.

Education

Masters Degree in Nursing or Physician Assistant Studies
