Job summary

Curana Health, Inc. is seeking Nurse Practitioners or Physician Assistants to lead proactive primary care for seniors in community settings. You’ll develop individualized care plans, conduct rounds, and collaborate with physicians and care teams to improve outcomes and reduce hospitalizations.

You’ll practice with autonomy, supported by leaders and advanced technology, while enjoying a flexible, mission-driven environment focused on dignity and quality of life for older adults.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with a focus on preventative care.
• Provide ongoing care management to improve quality of life and independence.
• Build meaningful patient relationships through regular rounds and team collaboration.
• Monitor health status and intervene early to prevent complications.
• Educate residents and caregivers on disease management and wellness strategies.
• Coordinate with physicians, staff, and families to develop comprehensive care plans.
