The role requires travel among facilities, autonomy in clinical decision-making, and support from a national team leveraging advanced technology for proactive care.

Qualifications

• PMHNP board-certified with ANCC certification.
• Active DEA license and state NP license in practice state.
• BLS and ACLS certified.
• Experience in primary care for seniors is preferred.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing or senior living communities.
• Perform health assessments and develop treatment plans.
• Manage acute and chronic conditions with focus on preventative care.
• Educate residents and families on disease management and wellness.
