Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long-term care members in nursing facilities. You will conduct comprehensive assessments, develop individualized care plans, and collaborate with a multidisciplinary team to improve member outcomes.

You will engage with families, physicians, and nursing home staff, advocating for patient-centered care and adherence to ISNP care models.

Qualifications

6+ months experience as a Nurse Practitioner or Physician Assistant.
Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program.
Active state NP or PA license and DEA license.
Active National provider number (NPI) and Medicare provider number.
Experience in nursing home environment strongly desired.
Excellent communication with physicians, members, and family members.

Responsibilities

Provides collaborative primary care to a panel of members.
Delivers ISNP Model of Care activities and advanced care planning.
Coordinates health plan clinical operations: prior authorization, Part A/B services, medication reviews, HEDIS/STARs quality gap closure.
Serves as trusted advisor to members, loved ones and nursing home staff.
Maintains frequent communication with members’ families and interdisciplinary care team.
