Job summary

Curana Health, Inc. is hiring Nurse Practitioners and Physician Assistants to lead proactive primary care in senior living communities. You will deliver comprehensive assessments, manage chronic illnesses, and collaborate with physicians and care teams to keep residents healthier at home.

This role involves travel between facilities and aims to reduce avoidable hospitalizations while providing autonomous, high-quality care with strong clinical and operational support.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification.
• Master's degree in nursing or physician assistant studies as applicable.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.

Responsibilities

• Deliver high-quality primary care in senior living communities.
• Develop treatment plans and manage chronic conditions.
• Build meaningful patient relationships and educate residents and families.
• Travel to and from assigned facilities for care delivery.
