Job summary

Curana Health seeks Nurse Practitioners and Physician Assistants to deliver proactive primary care in senior living communities. You’ll perform comprehensive assessments, develop individualized treatment plans, and manage chronic conditions to keep residents healthier and more independent.

Work with experienced physicians and care teams, with strong emphasis on autonomy, flexible scheduling, and innovative technology to support outcomes and patient relationships across multiple facilities.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to work in a team.

Responsibilities

• Perform comprehensive health assessments and develop individualized care plans for residents.
• Diagnose and manage acute and chronic conditions with emphasis on preventive care.
• Provide ongoing care management to improve quality of life and independence.
• Build relationships with residents, families and care teams during facility rounds.
• Monitor health status and intervene to prevent complications and hospitalizations.
• Educate residents and caregivers on disease management and wellness strategies.
• Coordinate care plans with physicians, staff, and family members.
• Maintain EMR documentation and ensure regulatory compliance.
