Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to provide primary care in senior living facilities. You will deliver direct patient care, manage chronic conditions, and coordinate with other healthcare providers to improve health outcomes for residents.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
• Masters Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
• Conduct regular rounds to assess residents’ conditions and adjust treatment plans.
• Collaborate with facility staff, family, and other healthcare providers to ensure comprehensive, coordinated care.
• Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses.
• Maintain accurate, timely, and thorough documentation within the EMR system, ensuring regulatory compliance.
• Participate in quality improvement initiatives to enhance care delivery and reduce readmissions.
