Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other providers to improve residents' wellbeing.

This role requires state licensure, a relevant masters, strong communication and teamwork, and willingness to travel among assigned facilities.

Qualifications

• Valid NP or PA license in the operating state and current board certification
• Master's degree in Nursing or Physician Assistant Studies
• Leadership, interpersonal and problem-solving skills
• Excellent written and oral communication abilities
• Punctuality, organizational skills, and ability to function in a team

Responsabilités

• Perform comprehensive assessments and care management for residents
• Conduct routine rounds and adjust treatment plans as needed
• Coordinate care with facility staff, families, and other providers
• Educate residents and families on disease prevention and wellness
• Maintain EMR documentation and ensure regulatory compliance
• Participate in quality improvement to enhance care and reduce readmissions
