Job summary

Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver proactive primary care in senior living communities. You will lead patient care, develop individualized plans, and collaborate with physicians and care teams to improve outcomes.

You’ll practice with greater autonomy, supported by experienced colleagues and advanced technology to identify needs and optimize care for older adults.

Qualifications

• Licensed in state of operation as NP or PA with current board certification.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal and problem-solving skills.
• Excellent written and oral communication abilities.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing or senior living settings.
• Diagnose, treat, and manage acute and chronic conditions with a focus on prevention.
• Build meaningful patient relationships and conduct facility rounds.
• Coordinate with physicians, staff, families, and care teams to plan care.
• Ensure accurate EMR documentation and regulatory compliance.
