Qualifications

• Must hold a valid NP or PA license in the United States.
• Master's degree in nursing or PA studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Ability to work well in a team and manage time effectively.

Responsabilités

• Deliver comprehensive primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose and manage acute and chronic conditions with a focus on preventative care.
• Build meaningful patient relationships with residents, families, and care teams.
• Collaborate with physicians and staff to coordinate comprehensive care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Support quality initiatives to reduce avoidable hospitalizations.
