Job summary

Curana Health seeks Nurse Practitioners or Physician Assistants to deliver proactive primary care directly in senior living communities. You’ll diagnose, treat, and manage conditions with autonomy and collaborate with physicians and care teams to improve outcomes.

You will travel to multiple facilities, support quality measures, maintain EMR documentation, and enjoy comprehensive benefits in a mission-driven organization.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships and conduct regular facility rounds with residents, families, and care teams.
• Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations.
• Educate residents and caregivers on disease management, preventative care, and wellness strategies.
• Collaborate Across the Care Continuum with physicians and care teams to coordinate comprehensive care plans.
• Maintain accurate and timely documentation within the EMR and ensure regulatory compliance.
