Job summary

Curana Health seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive primary care to seniors across skilled nursing and assisted living communities. You will work with physicians and care teams to develop individualized treatment plans and to improve outcomes while maintaining strong patient relationships.

Qualifications

• Valid NP/PA license in the state of operation.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and verbal communication skills.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with focus on preventive care and wellness.
• Maintain EMR documentation in a timely and accurate manner.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and families.
