Curana Health is seeking a Nurse Practitioner or Physician Assistant to lead primary care for seniors across multiple communities. You’ll diagnose, treat, and manage conditions with autonomy, supported by physicians and operational leaders.

Qualificações

• Valid Nurse Practitioner or Physician Assistant license in the state of operations and current board certification.
• Master's degree in nursing or PA studies required.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and verbal communication abilities.

Responsabilidades

• Perform comprehensive health assessments and develop individualized treatment plans for residents in multiple living settings.
• Diagnose, treat, and manage acute and chronic conditions with a focus on prevention and wellness.
• Provide ongoing care management to improve quality of life and independence.
• Conduct facility rounds and build relationships with residents, families, and care teams.
• Monitor health status and proactively intervene to prevent complications and hospitalizations.
• Educate residents and caregivers on disease management and preventive care.
• Collaborate with physicians, staff, and families to coordinate care plans.
• Serve as a clinical resource within the facility and participate in interdisciplinary discussions.
• Maintain accurate EMR documentation and ensure regulatory compliance.
