Job summary

Curana Health is seeking a Nurse Practitioner or Physician Assistant to provide primary care across senior living communities. You will conduct comprehensive assessments, create treatment plans, and collaborate with physicians and care teams in a mission-driven organization.

You’ll work with a supportive clinical and operational network, leveraging advanced technology to improve outcomes and enhance resident wellbeing in a flexible, purpose-driven environment.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships with residents, families, and care teams.
