Qualifications

• Current NP/PA license in state of operation.
• Master's degree in Nursing or PA Studies.
• Strong leadership, interpersonal, and problem-solving skills.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Perform comprehensive health assessments and develop individualized treatment plans.
• Diagnose, treat, and manage acute and chronic medical conditions with focus on preventative care.
• Provide ongoing care management to improve quality of life and independence.
• Build meaningful patient relationships with residents, families, and care teams.
• Collaborate with physicians, clinical staff, and facility leadership to coordinate care.
