Job summary

Curana Health is recruiting Nurse Practitioners and Physician Assistants to deliver proactive primary care to seniors in skilled nursing and assisted living communities. You will practice with autonomy, collaborate with physicians, and use advanced technology to improve outcomes.

This national role involves travel to assigned facilities, a comprehensive benefits package, and opportunities for CME and career growth within a mission-driven organization.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans for residents.
• Diagnose, treat, and manage acute and chronic conditions with focus on prevention.
• Provide ongoing care management to improve quality of life and independence.
• Conduct regular facility rounds and build relationships with residents and families.
• Coordinate care with physicians, staff, and specialists to create comprehensive plans.
• Ensure accurate EMR documentation and regulatory compliance.
