Job summary

Curana Health is hiring Nurse Practitioners or Physician Assistants to lead proactive primary care for seniors across our community network. You will assess residents, develop care plans, and partner with physicians to deliver compassionate, autonomous care in skilled nursing, assisted living, and related settings.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Master’s degree in nursing or physician assistant studies.
• Strong leadership and communication skills.

Responsibilities

• Deliver high-quality primary care with comprehensive assessments and individualized plans.
• Build meaningful patient relationships and provide ongoing care management.
• Collaborate with physicians, staff, and families to coordinate care.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Support initiatives to improve patient outcomes and reduce hospitalizations.
