Job summary
Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living communities and skilled nursing facilities. You’ll provide comprehensive assessments, manage chronic conditions, and partner with physicians and care teams to coordinate treatment plans, while supporting an autonomous, mission-driven practice.

Qualifications
- Valid NP/PA license in the operating state and current board certification.
- Master's degree in Nursing or Physician Assistant Studies.
- Strong leadership, interpersonal, and problem-solving skills preferred.
- Excellent written and oral communication skills required.
- Punctuality, organizational skills, and teamwork are essential.
Responsibilities
- Perform comprehensive health assessments and develop individualized care plans for residents in various living communities.
- Diagnose, treat, and manage acute and chronic conditions with emphasis on preventative care.
- Provide ongoing care management to improve quality of life and independence.
- Collaborate with physicians, staff, families, and care teams to coordinate care.
