Job summary

Curana Health, Inc. is seeking Nurse Practitioners and Physician Assistants to deliver proactive primary care to seniors in skilled nursing, assisted living, and independent living communities nationwide.

You will build meaningful patient relationships, coordinate care with physicians and care teams, and use advanced technology to support clinical decision-making. This is a mission-driven role with strong clinical and operational support.

Qualifications

• Valid NP or PA license in state of operations.
• Master's Degree in Nursing or PA Studies.
• Strong leadership, interpersonal and problem-solving skills.
• Excellent written and oral communication skills.

Responsibilities

• Perform comprehensive health assessments and develop treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care.
• Provide ongoing care management to improve quality of life and independence.
• Collaborate with physicians, staff, facility leadership, and families to coordinate care.
• Maintain accurate EMR documentation and ensure compliance with standards.
