Job summary

Curana Health is seeking nurse practitioners or physician assistants to lead primary care for seniors in community and facility settings. You will provide proactive, patient-centered care with support from physicians and a tech-enabled platform.

Applicants should hold an NP or PA license, a relevant master's degree, and demonstrate strong teamwork and communication. Some travel between facilities is required as part of the role.

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations.
• Current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Perform comprehensive health assessments and develop individualized treatment plans.
• Diagnose, treat, and manage acute and chronic conditions with preventative focus.
• Build meaningful patient relationships and conduct regular facility rounds.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and family members to coordinate care.
• Maintain accurate EMR documentation and ensure regulatory compliance.
