Job summary

Optum Home & Community Care, part of the Optum family, is expanding the Senior Community Care program to coordinate comprehensive clinical care for high-risk plan members in nursing homes, senior housing and home settings.

As an Advanced Practice Clinician within SCC, you will conduct comprehensive assessments, support short-term rehab, ensure accurate documentation, and collaborate with the interdisciplinary care team to advance patient outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted licensure in the practicing state without disciplinary actions.
• Current, active DEA licensure/prescriptive authority or ability to obtain per state regulations.
• Ability to obtain a collaborative practice agreement where applicable.

Responsibilities

• Conduct comprehensive assessments, including history, meds reconciliation, vitals and exam.
• Support members during short-term sub-acute rehab episodes.
• Ensure accurate documentation including ICD-10 conditions.
• Collaborate with the interdisciplinary care team and conduct advanced care planning.
