Job summary

Optum Home & Community Care, part of the Optum family, is expanding its Senior Community Care (SCC) program to coordinate clinical care for high-risk members in long-term care settings. The Advanced Practice Clinician role focuses on comprehensive assessments, medication reconciliation, and collaboration with an interdisciplinary team to deliver patient-centered care.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs; active and unrestricted licensure where practicing.
• Active and unrestricted license without disciplinary actions; ability to obtain license by start date.

Responsibilities

• Conduct comprehensive assessments including medical history review, vitals, and physical exams.
• Support members during short-term sub-acute rehabilitation.
• Ensure accurate documentation including ICD-10 codes.
• Collaborate with interdisciplinary care team; participate in planning.
• Conduct advanced illness and end-of-life discussions; provide counseling and education to patients and caregivers.
