Qualifications

• Graduate of an accredited NP or PA program with licensure to practice in Oregon.
• Board certification in relevant specialty (ANP/ACNP/AGNP or PA-C with NCCPA).
• Ability to obtain required state regulatory credentials before start date.
• Strong documentation and CMS-compliant charting skills.

Responsibilities

• Conduct comprehensive assessments and develop care plans.
• Manage acute events and chronic condition exacerbations with interventions.
• Maintain accurate CMS-compliant documentation and patient records.
• Collaborate with interdisciplinary care team to coordinate transitions.
• Lead advanced care planning conversations with patients and families.
• Provide counseling and education to patients and caregivers.
