Job summary

Curana Health seeks Nurse Practitioners and Physician Assistants to provide proactive, relationship-based primary care to seniors where they live. You’ll build meaningful patient relationships, improve outcomes, and reduce hospitalizations through our innovative care model and advanced technology.

Qualifications

• Nurse practitioner or physician assistant license in state of operations with current board certification.
• Masters degree in Nursing or PA Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication; punctuality and organization.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with focus on preventative care.
• Provide ongoing care management to improve quality of life.
• Build meaningful patient relationships with residents, families, and care teams.
• Collaborate with physicians, clinical staff, facility leadership, and family members to coordinate care.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in interdisciplinary care discussions.
