Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in diverse communities. You’ll work with experienced physicians and care teams, providing autonomous clinical leadership and high-quality care.

Travel to and from multiple facilities is required, with support from a mission-driven organization leveraging advanced technology, predictive analytics, and AI-powered tools to improve outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver comprehensive primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships with residents, families, and care teams.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and families to coordinate care plans.
• Maintain accurate documentation in the EMR and ensure regulatory compliance.
