Curana Health, Inc. seeks Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in our communities. You will develop treatment plans, manage chronic conditions, and collaborate with physicians and care teams to improve outcomes and reduce hospitalizations.

Competenze

• Valid nurse practitioner or physician assistant license in state of operations.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Ability to function well in a team and demonstrate punctuality and organization.

Mansioni

• Deliver comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships with residents, families, and care teams.
• Monitor health status changes and proactively intervene to prevent complications and hospitalizations.
• Educate residents and caregivers on disease management and wellness strategies.
• Collaborate with physicians, clinical staff, facility leadership, and family members to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory/compliance standards.
