Curana Health, Inc. is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in skilled nursing and senior living communities.

You will work with physicians, care teams, and families to improve outcomes and reduce hospitalizations while enjoying autonomy and support from a mission-driven organization. This role involves travel between assigned facilities, opportunities for professional growth, and a comprehensive benefits package.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver High-Quality Primary Care: perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships: conduct regular facility rounds and develop strong relationships with residents, families, and care teams.
• Collaborate Across the Care Continuum: partner with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans.
• Drive Clinical Excellence: maintain accurate documentation in EMR and ensure compliance with regulatory standards.
