Job summary

The Institute on Aging in San Mateo, CA, seeks a Care Manager II to conduct comprehensive assessments and manage individualized Community Living Plans for clients with complex needs.

Job description

The Care Manager II (CMII) at IOA conducts comprehensive assessments of clients with multiple medical and psychosocial needs and develops and manages individualized Community Living Plans.

Responsibilities

• Conduct comprehensive assessments and ongoing re-assessments of clients, including psychosocial, physical, mental health, environmental and spiritual needs.
• Write detailed assessment reports and initiate client-centered Community Living Plans consistent with program guidelines.
• Perform home, acute hospital, and skilled nursing facility visits; escort clients to medical appointments as clinically indicated.
• Identify, arrange for, and monitor appropriate community services based on knowledge of Medicare, Medi-Cal and other entitlement programs.
• Establish and maintain a care‑management relationship with clients and their informal support network with respect, dignity and support.
• Provide crisis intervention, advocacy, problem‑solving and therapeutic interventions.
• Meet with clients at least monthly (more often as clinically indicated) and review and modify the Community Living Plan continuously.
• Document all case management activities within 24–48hours, adding new problems to the plan as needed.
• Maintain required paperwork and a clear, concise charting system for continuity of care.
• Evaluate client needs for purchase of services and follow up to ensure timely delivery.
• Educate clients and their support networks about available resources.
• Maintain open communication with community providers, including physicians and other health‑care and social‑service workers.
• Monitor the quantity and quality of services provided by other providers.
• Coordinate placement and, when appropriate, terminate services in collaboration with the client, caregiver and involved services.
• Participate in research studies, data collection and Continuous Quality Improvement activities.
• Attend team and program meetings, contribute to problem‑solving initiatives, and provide updates on caseload activity.
• Adhere to ethical and legal standards of the National Association of Social Workers and regulatory requirements of the Institute on Aging.
• Complete continuing education and in‑service training to enhance case‑management, gerontology, family and community systems knowledge.
• Perform any other reasonably related duties assigned.

Education

• M.S.W. (Master of Social Work) or equivalent Master’s degree such as MPH, MFT, or a specialized degree with additional or specialized work experience in psychology, counseling or geriatrics.
• In lieu of a Master’s degree, a BA or BS in Social Work or a related field plus a minimum of two (2) years of relevant social‑work experience in conceptualizing and formulating biopsychosocial assessments, identifying care needs and executing effective interventions.

Background and Experience

• At least one year of experience working with disabled or older adults.
• In-depth understanding of medical and psychosocial problems of functionally impaired adults and older adults.
• Experience with individuals who have mental and/or behavioral health diagnoses and substance‑abuse disorders is highly desired.
• Exceptional communication, presentation and case‑management skills in community health‑care delivery.
• Detail‑oriented with strong problem‑solving and prioritization skills.
• Computer literacy required.

Compensation

Annual range: $85,759 – $102,495. This reflects base salary only and does not include additional equity, benefits or other non‑monetary compensation.
