Job summary

Institute on Aging is seeking a Care Manager I (CMI) in California to provide comprehensive case management for older adults and adults with disabilities. You will assess needs, plan, monitor services, and guide clients toward community resources, aiming to maintain independence and quality of life.

Responsibilities include home visits, care plan updates, coordination with providers, and ongoing documentation.

Qualifications

• One year working with disabled adults and/or older adults.
• Experience with medical and psychosocial problems of functionally impaired adults.
• Experience with mental and/or behavioral health diagnoses and substance abuse disorders desired.
• Exceptional communication and presentation skills relating to older adults, their support systems and teams of health professionals.
• Demonstrates case management skills and experience in the community health care delivery system.
• Detail oriented with good problem solving skills and the ability to prioritize multiple tasks.
• Computer literacy required.
• Bilingual English/Spanish required

Responsibilities

• Conducts comprehensive assessments and on-going re-assessments of the client, including psychosocial, physical and mental health, environmental needs.
• Writes comprehensive assessments and develops the Community Living Plan with the client.
• Conducts home visits and escorts clients to medical appointments as clinically indicated.
• Identifies, arranges for and monitors appropriate community services and entitlement programs.
• Establishes and maintains a care management relationship with clients and significant others.
• Meets with clients at least monthly and updates care plans as needed.
• Documents case management activity in progress notes within 24–48 hours.
• Maintains required paperwork and a clear charting system for continuity of care.
• Collaborates with team to evaluate clients’ ability to remain safely at home.
• Participates in data collection and Continuous Quality Improvement.
• Attends meetings and promotes open communication within the team.
