Job summary

Human Services Association is seeking a community-based care manager to assess, plan, and coordinate services for at-risk seniors, promoting independence and delaying institutionalization. You will manage a caseload, conduct home visits, and ensure documentation meets program requirements.

The role requires a BA in a related field and at least two years of elderly care experience, along with a California driver’s license and reliable vehicle for client visits.

Qualifications

• Bachelor's degree in Social Work or related field as required.
• Two years direct experience with elderly populations.
• Valid California driver's license and own vehicle.
• Ability to perform in-home assessments and develop care plans.
• Strong documentation and compliance with program requirements.

Responsibilities

• Conduct in-home psychosocial assessments and develop individualized care plans.
• Manage a caseload of 50–52 clients.
• Perform home visits and monitor progress.
• Maintain monthly care management documentation.
• Coordinate and verify services identified in the care plan.
• Maintain confidentiality and collaborate with the NCM.
