Job summary

Institute on Aging Southern California, LLC is seeking a dedicated social services professional to conduct comprehensive assessments and develop client-centered care plans for aging adults and those with disabilities. You will coordinate with Medicare/Medi-Cal and community providers to ensure seamless services, document thoroughly, and support clients and families.

Required BA/BS in Social Work and at least one year of relevant experience; bilingual abilities (English plus Cantonese and/or

Qualifications

BA/BS in Social Work or related field required.
One year experience with disabled adults or frail elderly.
Bilingual preferred: English plus Cantonese and/or Mandarin and/or Spanish.
Strong communication and presentation skills for frail elderly and teams.
Caseload management and knowledge of community health delivery.

Responsibilities

Conduct comprehensive assessments, including psychosocial and health needs.
Develop and implement the client-centered Community Living Plan.
Coordinate with Medicare, Medi-Cal and other programs.
Maintain charting and progress notes within 24 hours.
Educate clients about resources and train self-management when possible.
Collaborate with physicians and social service workers to ensure care continuity.
Participate in Continuous Quality Improvement and research data collection.
