Qualifications

• Master's degree in social work from an accredited school.
• Washington Clinical Independent Social Worker License required upon hire.
• 6 years of experience in Case Management.
• 1 year of Supervisory/management experience, preferably in Case Management in an acute care setting.
• Experience in problem-solving and conflict resolution is required.
• Experience in providing adult education to multi-disciplinary staff.

Responsibilities

• Oversee day-to-day Care Management activities and staff.
• Coordinate care, case management, discharge planning, and utilization management.
• Provide social work services and pre-admission case management.
• Serve as liaison among patients, families, physicians, and community partners.
• Establish and monitor clinical standards to assure quality and financial integrity.
