Job summary

Providence Regional Medical Center Everett is seeking a Manager RN Case Management responsible for daily management and organization of Case Management Department activities, including care coordination, discharge planning, utilization management, social work services, and pre-admission case management.

You will act as liaison among patients, families, staff, physicians, and external partners, and establish and monitor best practices to assure quality care and financial integrity.

Qualifications

• Master's degree in business/social work/nursing/healthcare related field from accredited school.
• Upon hire: Washington Registered Nurse License
• 6 years of experience in Case Management.
• 2 years of Supervisory/management experience, preferably in Case Management.
• Previous nursing experience within a variety of complex client care areas.
• Experience in problem-solving and conflict resolution.
• Experience in providing adult education to multi-disciplinary staff.

Responsibilities

• Manage daily activities and staff within the Case Management Department.
• Coordinate care coordination, discharge planning, utilization management, social work services, and pre-admission case management.
• Serve as liaison between patients, families, department staff, physicians and the healthcare team, community agencies, managed care representatives, skilled nursing facilities, and community agencies.
• Establish, implement and monitor best practice and clinical standards to assure quality patient care and financial integrity.
