Job summary

Providence Regional Medical Center Everett is seeking a Manager Care Management to lead the daily operations of the Care Management Department, including care coordination, discharge planning, utilization management and social work services. The Manager acts as a liaison between patients, families, staff, physicians and community agencies.

Qualifications

• Master's degree in social work from an accredited school.
• Washington Clinical Independent Social Worker License at hire.
• 6 years of experience in case management.
• 1 year of supervisory/management experience in case management in an acute care setting.
• Experience in problem-solving and conflict resolution is required.
• Experience in providing adult education to multi-disciplinary staff.

Responsibilities

• Oversee daily management and organization of the Care Management Department.
• Coordinate care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management.
• Act as a liaison between patients, families, department staff, physicians, and community agencies.
