Qualifications

Responsibilities

• Oversee daily management and organization of all activities and staff within the Care Management Department, including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management.
• Serve as liaison between patients, families, department staff, physicians, healthcare team, community agencies, managed care representatives, skilled nursing facilities, and other providers.
• Establish, implement and monitor best practices and clinical standards to ensure quality patient care and financial integrity of the organization.
