Job summary

Ascend Medical is hiring a Care Coordinator- Licensed Practical Nurse (LPN) in Woodstock, GA. The role focuses on supporting a healthcare provider visiting senior care communities, acting as a real-time scribe and hub for communication with facilities, families, and POA.

The ideal candidate has 2+ years of nursing experience in primary/geriatric care, current LPN registration, and proficiency with EMRs (Athena). Hybrid schedule with four days onsite and one remote day.

Qualifications

• 2+ years of nursing experience in a primary care setting, preferably geriatric
• Current LPN license/registration
• Experience documenting care and coordinating with providers

Responsibilities

• Act as primary scribe for the provider during patient visits and document in Athena EMR in real‑time
• Record patient encounters, including history, exam findings, assessment, plan, meds, and follow-up
• Update patient records promptly ensuring continuity of care
• Monitor patient progress and document changes in medications and care plans
• Assist in coordinating follow-up visits, changes in referrals, and communications with families
