Job summary

Summit Spine and Joint Centers in Lawrenceville, GA is seeking a full-time Licensed Practical Nurse (LPN) to join our Chronic Care Management (CCM) team. You will conduct outreach, follow-ups, document in EMR, and coordinate care for patients with chronic conditions.
