Job summary

Ascend Medical is seeking a Care Coordinator - Licensed Practical Nurse (LPN) to support providers in Senior Care Communities. You’ll document visits in Athena EMR, coordinate between facility staff, patients, and families, and help maintain continuity of care for geriatric patients.

The role requires 2+ years of nursing in a primary care setting with a focus on geriatrics, valid LPN license, and strong communication. This hybrid position offers predictable hours and a supportive care team.

Qualifications

• 2+ years of nursing experience in a primary care setting, preferably geriatrics.
• Current LPN license/registration.
• Experience documenting care and coordinating with care teams.

Responsibilities

• Act as primary scribe for the provider during patient visits, documenting in Athena EMR in real time.
• Record patient encounters, history, exam findings, assessment, plan, meds, and follow-up.
• Update records for continuity of care and track patient progress over time.
• Coordinate with facility staff and families to ensure timely care and clear communication.
• Assist in managing the provider's daily schedule and appointments.
