Job summary

Archway Transitional Care in Georgia is seeking a qualified LPN to join our nonprofit care team. You will facilitate communication, contribute to comprehensive care plans, and work with an interdisciplinary team to deliver high-quality patient care.

Qualifications

• Nursing degree or LPN program and active GA LPN license.
• Proficiency with computers and MS Office.

Responsibilities

• Facilitates communication to develop effective assessment/care plans.
• Establishes open lines of communication with consultants and interdisciplinary team.
• Assists Director with patient/family interviews and data for assessments and care plans.
• Ensures care plans meet regulatory timelines and approvals.
