Job summary

The Wright Center for Community Health is seeking a Licensed Practical Nurse (LPN) Care Coordinator to support outpatient care. You will coordinate care transitions from facilities, manage high‑risk patient registries, and handle medication tasks while communicating with primary care providers.

Responsibilities include follow-up visits, data management on symptoms and medications, and maintaining HIPAA-compliant processes. Weekend and holiday rotations are part of the schedule.

Qualifications

• Active Pennsylvania Practical Nursing license required.
• BLS Certification required.
• Experience with electronic health records (Medent a plus).

Responsibilities

• Oversee care for patients transitioning from facilities, with follow-up calls and coordination with Primary Care Team.
• Coordinate care for high-risk patients using risk stratification tools and registries.
• Arrange follow-up visits and manage patient data on symptoms, medications, and support.
• Handle clinical tasks including drawing up medications for clinicians and monitoring devices, while upholding HIPAA confidentiality.
• Field incoming patient calls, create orders, relay test results, manage medication refills and scheduling.
