Job summary

One Senior Care is seeking aParticipant Enrollment Specialist to guide seniors and families through joining our program. You will build relationships, complete assessments, coordinate documentation, and support each prospective participant through enrollment, working with our Interdisciplinary Team to ensure a smooth experience.

Ideal candidates are compassionate, organized, and able to manage multiple moving parts while meeting deadlines.

Qualifications

• Bachelor’s degree preferred.
• Experience working with frail or elderly populations is a plus but not required.
• Knowledge of Medicare and Medicaid regulations is helpful.
• Experience with community outreach, enrollment, sales, or marketing is a plus.

Responsibilities

• Meet with potential participants and families to explain the program and assess needs.
• Complete in-home and community-based assessments for prospective participants.
• Coordinate required evaluations, documentation, and intake steps.
• Attend intake appointments and support participants throughout the enrollment process.
• Complete intake and enrollment packets accurately and on time.
• Serve as the main point of contact for referrals and guide them through each stage of enrollment.
• Present referrals to the Interdisciplinary Team for review and ongoing collaboration.
• Maintain accurate records and communication updates in the CRM and EMR.
• Work closely with the Medicaid Enrollment Specialist to support a smooth transition to Medicaid processing and verification.
• Participate in team meetings, training, and quality improvement activities.
• Track weekly and monthly goals, enrollment progress, and key performance metrics.
• Help identify barriers to enrollment and work with the team to create solutions.
