Qualifications

• Strong communication, assessment, organizational, and problem-solving skills.
• Knowledge of healthcare regulations and community resources.

Responsibilities

• Conduct psychosocial, functional, and discharge assessments.
• Collaborate with physicians, care teams, and social services to develop discharge plans.
• Manage all aspects of discharge planning and address barriers to timely discharge.
• Facilitate communication among care team members to promote efficient care progression.
• Document assessments, care plans, and interactions in medical records.
• Mentor and precept new staff and interns.
• Support professional development and comply with policies and standards.
