Guardian Angels Senior Services in Elk River, MN seeks a Clinical Nurse Coordinator (TCU) to lead resident care in a high-acuity rehabilitation unit. You will develop individualized care plans and educate residents, families, and staff while coordinating tasks across the nursing team.

The ideal candidate has a Minnesota RN license, 5+ years RN experience including geriatrics, and supervisory experience. On-call rotation and full-time exempt status are involved.

Qualifikationen

• Minnesota RN license required (unrestricted).
• Five+ years RN experience, with at least 1 year in geriatrics.
• 3–5 years supervisory experience preferred.
• Current CPR certification required.
• Ability to work autonomously or collaboratively as situation dictates.
• Excellent verbal and written communication skills.

Aufgaben

• Coordinate, oversee, and provide resident-related care.
• Manage and delegate tasks to nursing staff on the unit.
• Work with the DON to ensure compliance with state and federal regulations.
• Notify attending physicians and family of significant changes and follow up as needed.
• Monitor in-house transfers and assist with discharge planning.
• Maintain current and accurate documentation and paperwork.
• Display courtesy and respect for residents, families, and staff.
