North Ridge Health and Rehab in Minneapolis, MN seeks an experienced Registered Nurse (RN) to join as a Nurse Manager for the Transitional Care Unit. Lead our nursing team, develop care plans, and coordinate interdisciplinary care to deliver exceptional resident outcomes.

Qualifications

• RN licensure with the MN Board of Nursing.
• Current CPR certification.
• Ability to maintain confidentiality and comply with HIPAA regulations.
• Desire to work with the elderly.
• Experience in medical, surgical, or geriatric nursing (preferred).
• Strong leadership and communication skills (preferred).
• Ability to work effectively in a fast-paced environment (preferred).

Responsibilities

• Provide nursing leadership and support to our nursing team, ensuring that our residents receive exceptional care and attention.
• Develop, implement, and evaluate nursing care plans to meet the unique needs of each resident.
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
• Monitor and report changes in resident condition, including signs of decline, abuse, or neglect.
• Maintain accurate and up-to-date records, including progress notes, care plans, and medication charts.
• Ensure compliance with regulatory guidelines, facility policies, and procedures.
• Mentor and support new staff members to ensure a smooth transition into their roles.
