Job summary

Griswold Home Care is seeking a compassionate Client Care Coordinator in Sussex and Kent Counties to manage client relationships, oversee service schedules, support caregivers, and ensure clear communication between clients, families, and the office team.

You will match caregivers to client needs, conduct intakes and follow-ups, and maintain accurate records while delivering high-quality, empathetic care to seniors and families.

Responsibilities

• Support caregivers with updates, training reminders, and problem-solving.
• Maintain accurate client records and documentation.
• Assist in resolving scheduling challenges or client concerns in a timely, professional manner.
• Collaborate with the office team to ensure smooth operations and excellent service.
